Acupuncture Treatment for Tension-type Headache


Nowadays Acupuncture Treatment is applying for tension-type headache widely.

At 2009, in Cochrane review,authors conclude that acupuncture could be a valuable non-pharmacological tool in patients with frequent episodic or chronic tension-type headaches. Besides, at 2012 in U.K. guideline "Consider a course of up to 10 sessions of acupuncture over 5~8 weeks for the prophylactic treatment of chronic tension-type headache"

The research that conducted in Australia, 2013 gives us which acupuncture treatment is effective for Tension-type headache, so I'm going to introduce to you.

Factors associated with conflicting findings on acupuncture for tension-type headache:Qualitative and quantitative analyses

Abstract

OBJECTIVES:

This study aimed to identify the factors that might have contributed to the conflicting outcomes about the efficacy of acupuncture for tension-type headache (TTH) through systematically reviewing relevant randomized controlled trials.

METHODS:

Thirteen (13) databases were searched from their inception until August 2010. There were no restrictions on language or year of publication. Included studies were randomized controlled trials comparing real with sham acupuncture, with patient selection guided by the International Headache Classification, and reported headache days. Meta-analyses and subgroup analyses were undertaken to compare the effects of real and sham acupuncture interventions and the effects of acupuncture with various needling techniques and treatment modes.

RESULTS:

Forty-three (43) studies were retrieved for further assessment from 120 potential studies. Finally, five studies of high methodological quality were included in this review. Standard mean difference (SMD) of the included studies showed no statistical significance between real and sham acupuncture (-0.31; 95% confidence interval [CI] -0.72 to 0.09), however, the heterogeneity among the studies was high (I(2)=81%). Subgroup analyses reduced heterogeneity, and showed that electro-acupuncture (SMD-1.60; 95% CI -2.33 to -0.88) to be more efficacious than manual acupuncture (SMD -0.13; 95% CI -0.41 to 0.14); needle retention with 30 minutes (SMD-0.46; 95% CI -0.87 to -0.06) being better than no needle retention (SMD 0.45; 95% CI -0.11, 1.01); and twice-a-week treatment (SMD -0.46; 95% CI -0.87 to -0.06) was better than once-a-week treatment (SMD 0.45; 95% CI -0.11, 1.01).

CONCLUSIONS:

Acupuncture stimulation mode, needle retention, and treatment frequency could be important factors contributing to the outcome of acupuncture for TTH. Further studies are warranted to determine treatment parameters to ensure effective translation of RCTs outcomes of acupuncture for patients with TTH.

Acupuncture Treatment is an effective method for Migraine patients.

The numbers of migraine patient is growing every year. It affects patient's ordinary life. sometimes, It reduces the quality of life seriously.

Now I'm going to show some good news to the people who has been suffering migraine for a long time.

CONCLUSIONS:

TA was the only treatment able to provide a steady outcome improvement in comparison to the use of only Rizatriptan, while RMA showed a transient placebo effect at T1.

CONCLUSION:

Our data show a lower pain intensity and lower Rizatriptan intake at six-months follow-up with no adverse events in acupuncture patients compared to those treated with valproic acid.

CONCLUSION:

The comprehensive auricular acupuncture reduces apparently the headache score for migraine without aura and relieves the clinical symptoms of migraine. It is the simple and effective therapy for migraine without aura.

  These results, though preliminary, are quite promising in supporting the effectiveness of ear acupuncture for treatment of migraine without aura.

How Fear of acupuncture affect to patient?

Some patients have fear of needles or needling, as known as needle phobia.
If you have needle phobia. You'd better avoid acupuncture treatments. Because It can over activate sympathetic nerve. so needling can cause dizziness, muscle contraction, hyperventilation.

This study was conducted in South Korea, 2012. and It proved fear of needle over-activates sympathetic nerve.

Red flag signs in headache patients.

Headache is not just a headache, and to distinguish the case to show the warning signs ," it hurts a lot of head simply","Some nausea and vomiting",  "the front sight is  black". Not be determined in such conditions. Once you let us know the dangerous situations.
1. (ICH, SAH) cerebral hemorrhage


In the case of cerebral hemorrhage, because it is a  bleeding of the brain. ICH(Intra-Cerebral Hemorrage), it is not accompanied by only a simple headache. Lowering of consciousness , half body paralysis, speech impairment, gait disturbance becomes the main symptom, the headache for just only comes with it is, so, there is little if you can not be distinguished.


But the problem is SAH, SAH is not a cerebral parenchyme's bleeding. Important point is a huge headache.


Simple tension headache and migraine, the patients knows very well.  Because the pain to come on a regular basis almost, the degree of pain is determined, what kind of medicine help this symtom quickly.

In the case of SAH, Features exploding emesis (vomiting due to the rapid rise of the cerebral pressure. Comes out like a fountain soil), 
a huge headache( you haven't  experienced up to now) .
 
Head hurts suddenly and severely, exploding vomitting  without any gastrointestinal symptoms such as diarrhea. If the patient came to clinic with the headache that they have not experienced even once until now, there is no reaction to every treatment . This simply means that it is a situation that must be transfered to specialized  care institutions
2. Brain tumor


In the case of brain tumors, there is a feature associated with cell differentiation and site of occurrence of each. First, I want to describe about childhood cancer.


(1) medulloblastoma (cerebellum tumor)


It is a childhood cancer. If medulloblastoma from adult occurs, it is possible to position rather than the cerebellum, to come to other parts of the body, in this case, is called the neuroectodermal tumor.

 For medulloblastoma response good to chemotherapy and radiation therapy. the 10-year survival rate is more than 50%

Since occur in the cerebellum, as well as headache, involves a severe dizziness. As a result of the tumor size increases, nausea and vomiting continued, and no response to all normal analgesic  Point of differentiation is a vertigo.


(2) encephalocystomeningocele


It is the most common brain tumor, which accounts for about 50% of all brain tumor patients. It is a positive, surgery goes well, the 5-year survival rate is more than 98%. Does not have a malignant tumor.

 
The encephalocystomeningocele can grow slowly in many cases, patients complain the continued headache continued. To distinguish from migraine is sometimes difficult.


Point of differentiation  is a "seizure" and "syncope".

 
Inspection is required if there is "convulsions (such as epilepsy)" or sometimes "syncope" is the patient of headache.


(3) astrocytoma

 
Astrocytoma tumor is typical that occur in adults. Brain tumor does not usually classify a Grade, but  astrocytoma is classified into four stages according to the degree of differentiation of the cells.


The first stage is positive, but it may be able to reoccur as a glioblastoma after treatment in many cases.


Grade III is referred to as a anaplastic astrocytoma,Grade IV is called  glioblastoma. The medulloblastoma, the survival rate is over 50% in 10years,  median survival values ​​are also long. 

Glioblastoma has 10 months of  median survival value.  The 5-year survival rate has been described in an article about 5-7%. 
Glioblastoma is about 50% of all malignant brain tumor.

This glioblastoma is <malignant tumor that grows rapidly>. In other words, if the headache came out first, it is intended that the tumor was the size of the already quite, does not react to any treatment because it is growing rapidly, headache will get even worse. You can can be accompanied by cramps and fainting according to the site that has occurred, . . If it occurs in the cerebellum, you may be accompanied by dizziness.


* Headache, such as crack might occur it when I get up in the morning
*  accompanied by nausea / vomiting and headache
* Problem is not solved even if  every treatment
* Frequency increases more, the intensity becomes strong
*  accompanied by seizure and fainting
* with language disorders and muscle weakness

Please see linked, These are summary of papers about plantar fasciitis. Plantar fasciitis is most common disease of heel pain. Because of it's severe pain it impact to ordinary life.
 Inspite of  the high prevalence of plantar heel pain, conventional treatments are still unsatisfied.

In my view, Acupuncture treatment combined with Physiotherapy or Injection treatment's success rate is higher than other treatment. and the most important thing is rehabilitation stretching!!

According to the research, integrative treatments(electro-acupuncture, herbal medicines, bee venom acupuncture) is effective in the treatment of Low back pain with leg pain, and warrants futher rigorous investigation.

Low back pain with leg pain, generally called Lumbar radiculopathy or HNP(Herniated Nucleus Pulposus) or Spinal disc herniation.

Acupuncture for Dry Eye Syndrome

This is the study to evaluate the effectiveness of acupuncture treatment for Dry Eye Syndrome.

This study was conducted in local hospitals in South Korea, and the patient's number was 150.

4 weeks of acupuncture treatment (bilateral BL2, GB14, TE 23, Ex1, ST1, GB20, LI4, LI11 and single GV23) These acupoints are located around eyes.
 

Acupuncture to treat nausea and vomitting in early pregnancy

RESULTS:

Women receiving traditional acupuncture reported less nausea (p < 0.01) throughout the trial and less dry retching (p < 0.01) from the second week compared with women in the no acupuncture control group. Women who received p6 acupuncture (p < 0.05) reported less nausea from the second week of the trial, and less dry retching (p < 0.001) from the third week compared with women in the no acupuncture control group. Women in the sham acupuncture group (p < 0.01) reported less nausea and dry retching (p < 0.001) from the third week compared with women in the no acupuncture group. No differences in vomiting were found among the groups at any time.

CONCLUSION:

Acupuncture is an effective treatment for women who experience nausea and dry retching in early pregnancy. A time-related placebo effect was found for some women.

 

Acupuncture is effective for rising pregnancy rate in IVF

The clinical pregnancy rate in the acupuncture group was significantly higher than that in the control and sham groups (35.7% vs 7.1% vs 10.7%; p=0.0169).

Acupuncture treated group-35.7%  of pregnancy rate.

Sham acupuncture( not using acupoints, insert needle randomly)-10.7%

CONCLUSIONS:

In this study, acupuncture and moxibustion increased pregnancy rates when used as an adjuvant treatment in women undergoing IVF, when embryo implantation had failed

Exercise for Plantar fasciitis


1.     Towel Stretch

Sit on a hard surface with your injured leg stretched out in front of you. Loop a towel around the ball of your foot and pull the towel toward your body, stretching the back of your calf muscle. Hold this position for 30 seconds. Repeat 3 times. When the towel stretch becomes too easy, you may begin doing the standing calf stretch.

2.     Standing Calf Stretching

Facing a wall, put your hands against the wall at about eye level. Keep the injured leg back, the uninjured leg forward, and the heel of your injured leg on the floor. Turn your injured foot slightly inward (as if you were pigeon-toed) as you slowly lean into the wall until you feel a stretch in the back of your calf. Hold for 30 to 60 seconds. Repeat 3 times. When you can stand comfortably on your injured foot, you can begin stretching the planter fascia at the bottom of your foot.

3.     Plantar Fascia stretch
          
Stand with the ball of your injured foot on a stair. Reach for the bottom step with your heel until you feel a stretch in the arch of your foot. Hold this position for 30 to 60 seconds and then relax Repeat 3 times. After you have stretched your bottom muscles of your foot. You can begin strengthening the top muscles of your foot.
4.     Frozen can roll
Roll your injured foot back and forth from your heel to your mid arch over a frozen can. Repeat for 3 to 5 minutes.This exercise is helpful if done first in the morning.
5.     Sitting Toe raise
Sit in a chair with your foot flat on the floor. Raise the toes and the ball of your injured foot off the floor while keeping your heel on the floor. Hold for 5 seconds. Repeat 10 times. Do three sets of 10.
6.     Towel pick up
With your heel on the ground. Pick up a towel with your toes and release. Repeat 10 to 20times.
7.     Resisted flexion, dorsal flexion, inversion, eversion with elastic band is recommended as well.

Tarsal Tunnel Syndrome

 What is Tarsal tunnel syndrome (TTS)?

Also known as posterior tibial neuralgia, it is a compression neuropathy and a painful foot condition in which the tibial nerve is compressed as it travels through the tarsal tunnel. This tunnel is found along the inner leg behind the medial malleolus.

What symptoms accompany a TTS?

1.     Numbness in the foot radiating to the big toe and the first three toes

2.     Pain, burning, electrical sensations, and tingling at the base of the foot / heel.

3.     The entire foot can be affected as different parts of the tibial nerve can become affected

4.     Ankle pain

5.     Possibilities of Inflammation or swelling. As the flexor retinaculum has a limited ability to stretch, increased pressure may eventually cause compression on the nerve within the tunnel causing the nerves to respond with altered sensations like tingling and numbness.

6.     Fluid could collect in the foot while standing or walking making the condition worse.
